Personal Choice Program
Living at home. Directing your care.

When you need assistance yet you want the comfort of living at home, Personal Choice offers you maximum independence. This program is designed to give seniors and those with long-term disabilities the option of managing their care.

Personal Choice enables you to purchase your care and services from a Personal Choice budget based on your functional needs. You’ll be in charge. You’ll hire and supervise your caregivers.

You’ll determine:

  • Who you want in your home
  • The most convenient times for your schedule
  • How your care attendant can best support your goals

Our staff will help you initiate these services. Your budget for a personal care attendant will be determined based on your functional needs. An “Individual Service and Spending Plan” focusing on specific goals will then be created.

Services may include support with activities such as:

  • Dressing
  • Bathing
  • Mobility
  • Daily routines

How do you know if you are eligible?

Personal Choice is available for individuals who want to return home or remain in their home.

To be eligible participants must:

  • Be age of 65 years or older, or individuals with disabilities who are over 18 years old
  • Possess the ability to self-direct all aspects of their personal care and community living needs, or have a representative who is available and willing to direct and manage their care and community living needs
  • Have a cognitive or physical disability that affects their ability to complete Activities of Daily Living in a safe or timely manner
  • Be eligible for Medical Assistance through The Rhode Island Department of Human Services Long Term Care
